The place opposite HO Hospital is very good - I get my shoe insoles there and they have absolutely everything there and will order anything you need.

If the disability is the result of treatment in HO Hospital - ie. an operation - there is a Social Services office opposite the main reception. 

If is is not the above there is a shop at the far end of the Car park again at HO Hospital - they have Wheel Chairs etc. but I don't know what there full range consist of ?
